PACE programs: all-inclusive care that keeps older adults at home
68 PACE programs with 148 verified care centers in 8 states. Each program lists only care centers confirmed on a state page or the program's own website, and the service area it publishes.
What is PACE?
PACE, the Program of All-Inclusive Care for the Elderly, is a Medicare and Medicaid program. A PACE organization provides all of a participant's medical care and long-term services, usually from a PACE center that combines a clinic, therapy, meals and day activities, plus care at home and transportation.
Who can join
- You are 55 or older.
- You live in the program's service area. Each program serves specific ZIP codes or counties; living near a center does not by itself make you eligible.
- Your state certifies that you need a nursing-home level of care.
- You can live safely in the community with PACE's help when you join.
Enrollment is voluntary and is arranged with the program, which confirms eligibility with the state.

Where this information comes from
Program names, Medicare contract numbers and enrollment come from the CMS Medicare Advantage Plan Directory. Care centers and service areas come from each state's PACE page or the program's own website, with the source and the date it was checked on every program page. A program's administrative or mailing address is never listed as a care center.
