Mcfarland Senior Outreach Services

Overview

The Senior Outreach Department provides a variety of services and programs for people over the age of 60. The two primary services we provide are case management services and nutrition services. Our case managers assist community based seniors navigate through a maze of information and identify resources to promote wellness and independence within their home and community. The nutrition program offers noon meals at our meal sites and through home delivered meals (in certain areas). Other services include health and wellness activities, educational programs and social groups. Outreach coordinates transportation services for shopping and the meal site along with RSVP rides for medical appointments. A medical loan closet is available for short-term use.
