We provide services to seniors, acute and chronically ill patients which can help prevent rehospitalizations, postpone institutionalization, and reduce the need for urgent care. We service ten counties in Georgia. Our services are skilled and non-skilled services that include but are not limited to: Assistance with activities of daily living (Bathing, Grooming, Dressing, Meal prep etc.) Wound care IV Therapy Injections Skilled Observation & Assessment Chronic Disease Management (Diabetes, COPD, CHF, HTN) Post-Surgical Care Post-Stroke Care Fall Injury Prevention Medication Teaching and Management Nutritional Counseling and Diet Teaching Companionship Care, Homemaking Care, Personal Care Blood work and Lab draws.
